Guidelines

           CEVC AGREEMENT & GUIDELINES   -   For Parents, Parent/Guardians & Additional Participants

I, _______________________________________________, understand that I will have the following Agreement & CEVC Guidelines for Parents, Parent/Guardians & Additional Participants explained to me by CEVC staff and my initials indicate that I agree that each were discussed with me and that I had the opportunity to ask questions and further discuss my questions or concerns.  My initials further indicate that I understand each item and I will comply with each item while using the CEVC supervised visitation or monitored exchange services.

1)       _____ I agree, understand and will comply in that all communication will be directed only to Theresa Amershek, CEVC Director.  This includes all written and verbal messages, schedule changes and cancellations, questions, concerns or issues.  If Theresa Amershek is unavailable I am expected to either leave a message on her voice mail or I may continue to call back until I reach her, whichever I prefer.  Leaving a message with another staff or requesting a return call other than on Ms. Amershek’s voice mail will not alleviate my responsibility to either leave a message on her voice mail or contact and speak with her directly.  The only exception to this is unforeseen delays or necessary cancellations after 5:00 pm.  Friday through Sunday evening for weekend and holiday supervised visitations or monitored exchanges.

 

2)       _____ I agree, understand and will comply with contacting Theresa Amershek directly or leave her a voice mail at least 24 hours before the scheduled visit or exchange if I cannot attend due to my own scheduling conflicts (i.e. vacation, scheduled work hours, meetings, trainings, etc.).  Scheduling conflicts that come up or occur on a holiday or after 5:00 pm Friday through Sunday evening for weekend visits or exchanges I understand that lack of such notice will require me to pay for the full scheduled time.  If it is an unforeseen circumstance, proof may be necessary for special consideration for the absence to be excused.

 

3)       _____ I agree, understand and will comply if in the event of illness on week day visits or exchanges I will contact Theresa Amershek directly or leave a message on her voice mail at least two (2) hours prior to the scheduled visit or exchange so that she will have ample time to contact the second party and cancel the exchange or visit.  For sudden illness on holidays or after 5:00 pm Friday through Sunday evening for weekend visits or exchanges I will contact the staff at least two (2) hours prior to the scheduled visit or exchange so that staff will have ample time to contact the second party and cancel the exchange or visit.  I understand that lack of such notice will require me to pay for the full scheduled time.  Exception for special consideration for excused absence may require a doctor’s note stating there was an emergency visit or medical necessity.

 

4)       _____ I agree, understand and will comply with not using alcohol, illegal drugs or abusing prescription drugs at least 24 hours before visiting or exchanging the child(ren).  I understand that the visit or exchange will be cancelled if I am suspected of having used or being under the influence of any of these substances.

 

5)       _____ I agree, understand and will comply with seeking prior approval from Theresa Amershek to bring anyone else to the visit or allow anyone else to transport the child(ren) for pick up.  Prior to the day of the visitation or exchange I agree I must provide my written request and designation of the person I am permitting to visit or pick up my child(ren).  Prior request is required so that there is adequate time for all necessary parties to be contacted and permission to be obtained or denied.  I understand sharing the parent and child visit time with others is discouraged, however, I further understand special circumstances will be considered and may be approved if all necessary parties agree and deem the visit with the additional visitor would serve the best interest(s) of the child(ren).  I further understand that during the visit or exchange bringing a family member, significant other or friend to wait inside the lobby or building is not permitted.

 

6)       _____ I agree, understand and will comply with arriving at the CEVC and depart from the CEVC at the designated times as outlined in the CEVC Guidelines for Parents, Parent/Guardians & Additional Participants.

 

7)       _____ I agree, understand and will comply with using the designated entrance/exit at all times.  Non-Residential parents and their approved additional participants are to always use the East door and the Residential Parent/Guardian and their approved additional participants are to always use the West door.  I will wait in the waiting area directly adjacent to my assigned entrance door, or as directed and outlined in the accompanying CEVC Guidelines for Parents, Parent/Guardians & Additional Participants.

 

8)       _____ I agree, understand and will comply with not bringing to the visit or exchange any weapons or articles (pocket knife, screwdriver, pen, etc.) that can be used as weapons.  I agree to a metal detection scan and /or search by CEVC staff to assure there are no weapons on my person.

 

9)       _____ I agree, understand and will comply with not bringing to the visit or exchange any devices that record by audio or video, this includes photo cameras, camcorders, VHS cameras, cell phones, lap top computers/notebooks, tape recorders, micro recorders, etc. as they are not permitted.  The only consideration for exception will be photo cameras for special events such as holidays or birthdays celebrated at the CEVC.  Prior to the day of the visitation or exchange that I wish to take pictures I understand my prior written request is required so that there is adequate time for all necessary parties to be contacted and permission to be obtained or denied.

 

10)   _____ I agree, understand and will comply that upon arrival for Supervised Visits I will give my keys, driver’s license, cell phone and any weapons considered to be or could be used as weapons to CEVC staff.  I understand at the end of the visit CEVC staff will return my possessions to me at the designated time for my departure.

 

11)   _____ I agree, understand and will comply with that I am not allowed to leave the premises with the child(ren).  The only exception to this is the rare circumstance as Court Ordered by a Judge and in coordination between the Court and Theresa Amershek, CEVC Director, and all necessary parties.

 

12)   _____ I agree, understand and will comply with commenting on or discussing any concerns or issues regarding the CEVC practice or procedures, court dates and proceedings, therapy or legal appointments, etc. of either an adult or business nature in the presence of the child(ren).  All visits and exchanges are to be positive, friendly, child-focused and interactive in conversation and play (i.e. their school activities and interests, instructions for medication, re-confirming schedules etc.).  Further I agree, understand and will comply with not making any promises to the child(ren) about future living arrangements or unsupervised visits.

 

13)   _____ I agree, understand and will comply with not using any profanity, physical punishment, or intimidating or threatening gestures or tone, or raised voice with the child(ren) or in the children’s presence at any time during the visits or exchanges.  I understand that if I do not comply that the visit or exchange will be immediately terminated.

 

14)   _____ I agree, understand and will comply in that I am responsible for addressing my children’s negative behavior or issues that may arise at arrival, during and departure from the visit or exchange.  I further agree that I will provide appropriate, non-threatening and non-violent parental guidance, response and interventions that are not physical, intimidating, threatening or abusive to my child(ren).  I understand that if I cannot effectively correct my child’s negative behavior or issue that the CEVC staff will guide or assist me with appropriate interventions and/or terminate the visit.

 

15)   _____ I agree, understand and will comply with not questioning the child(ren) about their other parent, foster parent, extended family members, friends, significant others or any other persons that the child(ren) are acquainted with.  This includes their whereabouts, address, relationships, activities (only the child’s activities and interests can be discussed), work, etc.  The focus of the visit is the relationship with the child(ren) and interactive conversation and play.  I understand that if I do not comply that the visit or exchange will be immediately terminated.

 

16)   _____ I agree, understand and will comply with not speaking negatively about the child(ren)’s other parent, foster parent, friends, extended family members, significant others or any other persons the child(ren) are acquainted with.  This also includes comments or conversation that is negatively persuasive or undermining of the child’s relationship with the other parent, foster parent, extended family members, friends, significant others or any other persons that the child(ren)  are acquainted with.  I understand that if I do not comply that the visit ore exchange will be immediately terminated.

 

17)   _____ I agree, understand and will comply with speaking in a tone at all times that is audible to both the CEVC staff and child(ren) and that I will not attempt to hide my questions or conversations with the child(ren) by whispering or in any other manner.  I understand that if I do not comply that the visit or exchange will be immediately terminated.

 

18)   _____ I agree, understand and will comply with not bringing correspondence, messages, or any other material or items to the visit or exchange for the CEVC staff or children to deliver to the other parent, foster parent, extended family member, friend, significant other or any other person the child(ran) are acquainted with.  The exception will be medication or instructions for the care or special needs of the child (ren).

 

19)   _____ I agree, understand and will comply that if I wish to give the child(ran) a birthday or holiday card or present that I must bring them unwrapped at the designated arrival time for the visit or exchange and they will be inspected for inappropriate correspondence, messages, etc.  After inspection, if the gift or card is deemed appropriate the gift or card may be wrapped in the presence of staff while waiting for the child (ren) to arrive and given to the child (ren) at the visitation or sent home with the child if to be opened later.

 

20)   _____ I understand that all communication, contacts, conversations, voice mails and messages, arrival & departure times, and assessments of the visits and exchanges are noted by CEVC staff.  The records maintained by the CEVC are not confidential and may be disclosed to the Court, Guardian ad Litem or CASA if appointed, law enforcement as appropriate, or SRS and Foster Care agencies for monitoring and case planning, etc.  CEVC records, notes and documentation are not provided to the CEVC participants.

 

 

21)   _____ I understand that CEVC staff reserves the right to immediately cease a visitation or exchange or terminate CEVC services if I display or verbalize any inappropriate behavior.  Violent, abusive or threatening behavior toward or in the presence of the child(ren) or CEVC staff or other families or guests of the CEVC will not be tolerated and will be immediately reported to law nforcement and/or SRS.  The Court, Attorneys, Guardian ad litem, CASA staff, Foster Care agency, etc. involved in the case will also be notified.

 

22)   _____ I understand that if the CEVC staff terminates the visit for any reason that the CEVC staff will retain care and custody of the child(ren) until the child(ren) is returned as appropriate to the Residential Parent or Guardian or Custodial adult or placed into police protective custody.

 

23)   _____ I further understand that CEVC staff may terminate its involvement in this case if I chronically or egregiously fail to adhere to the CEVC Agreement & Guidelines for Parent, Parent/Guardians & Additional Participants.
